Have the following on hand before you start. The form asks for each in turn.
- Your legal name, as printed on your passport.
- Country and city of residence.
- Date of birth.
- Your OnlyFans username, or "not yet" if you have not started.
- Your most recent month of OnlyFans revenue in USD. If you have not started, write zero.
- Your largest social platform, the handle, and approximate follower count.
- What kind of content you produce, in one sentence.
- Whether you are currently signed with another agency or manager. If yes, when that contract ends.
- Why you are applying to us specifically.
- Who referred you, if anyone.
